Supporting Healthier and Independent Living for Chronic Patients and Elderly
Ageing and changes in lifestyles are the main causes for the increasing prevalence of chronic disorders in Europe. This leads to further dysfunctions of healthcare systems such as redundant visits to healthcare facilities, for instance. In this context, there is a broad consensus on the need for reorganizing the delivery of care and social support services for chronic patients, with the support of Information and Communication Technologies (ICT). Indeed, many small scale pilot experimentations have provided very promising results during the last decade.
Yet, current fragmentation of health providers and community services together with the challenge of managing co-morbidity are the main obstacles to large scale deployment of integrated care services.
The objective of NEXES is the extensive deployment and sustainability of several integrated care services, representative of a broad spectrum of health problems, from those affecting citizens at risk or early stages of the disease to those characterising patients with advanced chronic disorders.
The services that NEXES will deploy are grouped in four programs:
- “Well being and rehabilitation”: this programme promotes healthy life-styles and active physical exercise in clinically stable chronic patients. ICTs contribute to enhance their self-management and to improve compliance with prescribed treatments.
- “Enhanced Care support”: this programme targets more specifically frail patients with advanced chronic diseases and several comorbid conditions and facing high risks of hospitalization.
- “Home hospitalisation & early discharge”: This program brings forward the potential of already existing home hospitalization services in patients with exacerbations of their chronic conditions by applying e-support.
- Support diagnostic and/or therapeutic procedures: primary care professional attending chronic patients benefit from increased accessibility to specialists and consultants.
These four programs are will be assessed in Catalonia, Central Norway Region and Athens with a total of 5000 patients, with the ultimate objective of being deployed in routine practice.
